Lung Cancer and Mesothelioma 25 Years After 9/11
September 11, 2026 marks 25 years since the terrorist attacks that changed the United States forever. As the nation remembers those who lost their lives, it is also important to recognize the thousands of first responders, construction workers, cleanup crews, volunteers, and other individuals who worked at and around Ground Zero in the days, weeks, and months that followed. Many of these workers were exposed to a dangerous mixture of dust, smoke, debris, and hazardous building materials, including asbestos, and because asbestos-related diseases can take decades to surface, some of those exposed in 2001 may only now be entering the period when illness connected to that exposure begins to appear.
The collapse of the World Trade Center released an enormous amount of dust and debris throughout Lower Manhattan. The buildings contained a wide variety of construction materials, and their destruction sent potentially hazardous substances into the surrounding air. Workers involved in rescue, recovery, demolition, debris removal, transportation, and cleanup may have inhaled or otherwise encountered these materials while carrying out their responsibilities.
Conditions in the immediate aftermath of the attacks were extraordinarily difficult. Many workers spent long hours in areas filled with heavy dust and debris, often while responding to urgent rescue and recovery needs. During the earliest stages of the response, respiratory protection and other safeguards were not always available, practical, or consistently used.
Why Asbestos-Related Diseases Take Decades to Develop
Asbestos is dangerous because of what happens after it is inhaled. The fibers are microscopic, sharp, and extremely durable. Once they reach the deep tissue of the lungs or the pleura, the thin lining surrounding the lungs, the body has no effective way to break them down or clear them out. Immune cells attempt to engulf the fibers and fail, and the fibers remain lodged in the tissue for the rest of a person’s life.
That permanent presence sets off a slow, ongoing cycle of irritation, inflammation, and scarring. Over many years, the repeated injury to surrounding cells causes genetic damage to accumulate. In some people, that damage eventually leads to asbestosis, in which scar tissue stiffens the lungs. In others, it leads to lung cancer or mesothelioma, a cancer of the pleura or abdominal lining that is almost exclusively linked to asbestos.
The reason this takes decades rather than months is that cancer does not begin with a single injury. A cell must acquire several separate mutations before it loses the ability to regulate its own growth, and each of those mutations is a rare event that occurs only after countless rounds of inflammation and cell repair. Even once a cancerous cell forms, a tumor grows silently for years before it becomes large enough to cause symptoms or appear on imaging. Early signs such as shortness of breath, chest pain, or a persistent cough are also easy to mistake for common conditions, which often delays diagnosis further. Because the harm builds gradually rather than occurring at the moment of exposure, these diseases typically are not diagnosed until 20, 30, 40, or even more years after the fibers were first inhaled.
That delay makes the 25th anniversary of 9/11 particularly significant from an occupational health perspective. Workers exposed to hazardous materials in 2001 may now be reaching the point when asbestos-related diseases are more likely to emerge. The passage of time can also make it harder to connect a current diagnosis with work performed many years earlier, especially for individuals who worked for multiple employers or at numerous job sites over their careers.
Who May Have Been Exposed
The potential for asbestos exposure was not limited to firefighters and other emergency responders. A broad range of workers participated in rescue, recovery, demolition, cleanup, transportation, and infrastructure restoration efforts following the attacks, including: construction workers, demolition crews, laborers, ironworkers, electricians, operating engineers, heavy equipment operators, truck drivers, utility workers, sanitation workers, transit employees, and volunteers.
Many worked directly with debris or in areas where dust remained present for extended periods; others transported materials away from the site or repaired damaged infrastructure in nearby buildings.
Exposure was not limited to the worksite itself. Residents, office workers, students, and others living or working in Lower Manhattan may also have been exposed to hazardous materials that traveled beyond the immediate disaster zone or settled inside surrounding buildings. Workers may also have unintentionally carried contaminated dust home on clothing, shoes, tools, or personal belongings, a form of secondary exposure long recognized as a concern in asbestos cases, particularly when workers regularly returned home wearing contaminated clothing.
Maintaining Exposure Records
For individuals who worked at or near Ground Zero, maintaining records related to employment and exposure history is important. Information such as employers, contractors, job titles, work locations, dates, duties, and the types of materials handled may help establish where and how exposure occurred if an asbestos-related illness develops later. Employment records, union records, medical files, photographs, coworker information, job-site records, and other documents may help reconstruct a person’s work history and potential exposure.
Even details that seem minor today may become valuable years later when determining how an illness may be connected to past occupational exposure.
Renewed Efforts to Strengthen Asbestos Protections
The 25th anniversary of 9/11 also comes during renewed federal efforts to strengthen protections against asbestos exposure. In August 2026, Senators introduced the bipartisan Alan Reinstein Ban Asbestos Now Act of 2026; legislation intended to address remaining gaps in federal asbestos regulation and further restrict asbestos use and exposure in the United States. While many asbestos-related illnesses diagnosed today stem from exposures that occurred decades ago, reducing current and future exposure remains essential to protecting workers.
